Usually uneven airflow — a closed or blocked vent, a duct run that's too long or poorly sized for that room, or a return-air imbalance. Sun exposure and poor insulation make it worse but are rarely the whole story on their own.
Ductwork is designed to deliver a certain amount of air to each room, and that balance gets thrown off by furniture blocking a vent, a partially closed damper somewhere in the system, or a duct run that was undersized for the room's distance from the furnace. Rooms far from the unit, above a garage, or with lots of west-facing windows are the classic offenders, but the fix is usually in the ductwork or dampers, not the equipment itself.
A duct evaluation identifies where the imbalance actually is rather than guessing — sometimes it's a simple damper adjustment, sometimes it's an added or resized duct run. Either way, it's worth diagnosing rather than just living with the room that's never comfortable.
